Party
Complete Section 2 only if you are
applying for joint credit, OR you are applying for individual credit but are relying on
income from alimony, child support, or separate maintenance or the income or assets of
another person as the basis for repayment of the credit requested. If you are married and live in a
community property state (Arizona, California, Idaho, Louisiana, Nevada, New Mexico,
Texas, Washington, Wisconsin) complete section B, providing information about your spouse.
By submitting this Credit Application,
I (1) make the attached representations, which are certified true, correct and
complete, for the purpose of obtaining credit, (2) authorize you to investigate my credit
record, to verify my credit, employment and income references and to gather such other
information that you consider necessary and appropriate, (3) authorize you to give credit
reporting agencies and others information regarding your credit experience with me, and
(4) understand that you will retain this application whether or not it is approved.
Notice to California Customers:
A married applicant may apply for separate account.
Notice to Ohio Customers:
The Ohio laws against discrimination require that all creditors make credit equally
available to all creditworthy customers, and that credit reporting agencies maintain
separate credit histories on each individual upon request. The Ohio civil rights
commission administers compliance with this law.
Notice to New York Customers: In
connection with your application for credit, a consumer report may be obtained which
contains information on your crdit worthiness, credit standing, credit capacity and
general reputation. If you request, you will be informed whether a consumer report
was obtained and if so, you will be given the name and address of the consumer reporting
agency which furnished the report. If yourappliation is granted, subsequent consumer
reports my be obtained in connection with any update, extension or renewal of credit.
Notice to Wisconsin Customers:
No provision of a marital property agreement, unilateral statement under section
776.59 Wisc. Stats. or a court decree under section 776.70 adversely affects the interest
of the creditor unless the creditor, prior to the time the credit is granted, is furnished
a copy of the agreement, statement or decree or has actual knowledge of the adverse
provision when the obligation to the creditor is incurred.
